You can review the information and enroll yourself and your family on this site. Click on the "Benefits" tab, review the plan options and the click the "Enroll Now" button to get started. Payment methods include Bank Draft/ACH and/or Credit Card.

ACCESS is KEY

One of the most overlooked componet of your Limited Medical Plan is the National Preferred Provider Network (PPO). You and your family will have ACCESS to hospital and physician discounts throughout the country. Having a national provider network allows you to stretch your indemnity benefit dollars and reduce your overall healthcare cost through negotiated discounts and per diem rates.

Most all of the programs you will find available to you are Guarantee Issue and require no health questions or underwriting.

Enrolling in yourself and your family can be accomplished in several ways:

1. Online, just click the Benefits tab at the top of this page and you are on your way,
2. Telephone, you can also enroll by phone by calling 1-866-971-2368 and finally
3. you can download the Enrollment Guide PDF and print the paper enrollment form and fax your application or turn into your Human Resources Department.
