Enrollment must be completed within 31 days of your date of hire or rehire.

Payroll deductions are made on a pre-tax, bi-weekly basis. Due to the pre-tax nature of the benefit plan, IRS regulations determine when and what changes may be made. Your benefit elections will remain in effect for the entire plan year, unless you experience a qualified family status change, for example marriage, divorce, adoption or birth of a child, or loss or gain of other coverage.

ACCESS to Healthcare is KEY

One of the most overlooked components of your Limited Medical Plan is the National Preferred Provider Network (PPO). You and your family will have ACCESS to hospital and physician discounts throughout the country. Having a national provider network allows you to stretch your indemnity benefit dollars and reduce your overall healthcare cost through negotiated discounts and per diem rates.
